Electrocardiography Changes after Stroke

Abstract
Electrocardiography (ECG) changes are common after stroke and are associated with increased mortality and morbidity. ECG changes are more common in patients with cerebral hemorrhage than in those with infarction. Neurogenic cardiac injury is related to brain injury-induced catecholamine and systemic inflammatory responses. The neurogenic stunned myocardium (NSM) syndrome caused by local release of norepinephrine from myocardial sympathetic nerve terminals, may also cause ECG changes, arrhythmia, elevated cardiac enzyme levels, and left ventricular dysfunction. It could sometimes cause fatal arrhythmia such as Torsades de pointes that leads to sudden cardiac death. Neurogenic cardiac abnormalities are often transient, lasting for several days. Management should focus on general supportive care and treatment of the injured brain.
